  • This survey is for people 62 years of age and older who live in Plainfield, New Jersey. 
  • This survey was designed to gather information from people like yourself about your living arrangements, health, and your feelings in general about your community.
  • The average length of time for answering all of the questions is 20 to 30 minutes.  Please fill it out in one session.  If you are unsure about how to answer a question, give your best guess.  ​
